This assessment resource page is divided into two sections.
The first section contains resources for students who would require minimal or moderate supports with achieving their post secondary goals. This category of students would include youth that would be typically classified has having “high incident” or “mild” disabilities and those having moderate disabilities.
The second half of this page contains examples of resources for youth who would require complex supports with achieving their post secondary goals. This category of youth would include students that would be typically classified has having significant disabilities.
Both sections are divided into subsections based upon the Post-Secondary Goal Areas (Education and Training, Employment and Independent Living). Included in these sections are examples of informal and formal assessments, internet links, and suggestions for portfolio usage.
Also included on this page is the Career Planning Begins With Assessment Guide. This comprehensive resource for Professionals Serving Youth with Educational and Career Development Challenges, was developed by the National Collaborative on Workforce & Disability for Youth, Institute for Educational Leadership, the Connecticut Transition Assessment Resource Manual; developed by the Connecticut Transition Task Force, 2008, and the Pittsburgh Public SD, Online Transition Assessment Guide.
